REASONABLE ACCOMMODATION

Sometimes people with disabilities may need a reasonable accommodation in order to take full advantage of the Lone Wolf Housing Authority housing programs and related services. When such accommodations are granted, they do not confer special treatment or advantage for the person with a disability; rather, they make the program accessible to them in a way that would otherwise not be possible due to their disability. This policy clarifies how people can request accommodations and the guidelines the Lone Wolf Housing Authority will follow in determining whether it is reasonable to provide a requested accommodation. Because disabilities are not always apparent, the Lone Wolf Housing Authority will ensure that all applicants/tenants are aware of the opportunity to request reasonable accommodations.

DIRECTIONS FOR HOW A PROSPECTIVE APLICANT MAY APPLY FOR HOUSING

Families wishing to apply for the Public Housing Program must complete an application for housing assistance. Applications may be made by filling out the Lone Wolf Housing Authority’s form application, and any required accompanying forms. The Lone Wolf Housing Authority will receive applications in person, by mail, email, or fax as specified below:

 

In Person:    901 Walker Circle, Lone Wolf, OK  73655-9725

                    On Monday, Wednesday, or Thursday between the hours of 9am and 3pm.

At all times or when the Housing Authority is closed, a sign will be posted in a manner visible to visitors outside the office (on the outside of the Housing Authority’s office door or similarly situated), to inform applicants how they may deliver an application when the Housing Authority is closed, including the methods above

 

Applications are taken to compile a waiting list. Completed applications will be accepted for all applicants and the Lone Wolf Housing Authority will verify the information.

 

Applications will be mailed to interested families upon request.

 

The completed application will be dated upon its return to the Lone Wolf Housing Authority.

 

Persons with disabilities who require a reasonable accommodation in completing an application may call the Lone Wolf Housing Authority to make special arrangements.

 

The application process will involve two phases. The first phase is the initial application for housing assistance or the pre-application. The pre-application requires the family to provide limited basic information establishing any preferences to which they may be entitled. The Housing Authority will check for any criminal history. This first phase results in the family’s placement on the waiting list.

 

Upon receipt of the family's pre-application, the Lone Wolf Housing Authority will make a preliminary determination of eligibility. The Lone Wolf Housing Authority will inform the family of the date of placement on the waiting list.  If the Lone Wolf Housing Authority determines the family to be ineligible, they will state the reasons therefore and will offer the family the opportunity of an informal review of the determination.

 

The applicant may at any time report changes in their applicant status including changes in family composition, income, or preference factors. The Lone Wolf Housing Authority will annotate the applicant’s file and will update their place on the waiting list.

 

The second phase is the final determination of eligibility, referred to as the full application. The full application takes place when the family nears the top of the waiting list. The Lone Wolf Housing Authority will ensure that verification of all preferences, eligibility, suitability and selection factors are current in order to determine the family’s final eligibility for admission into the Public Housing Program.
